Hi,
Are you referring to this form - http://www.jotform.us/form/52945000515144? Do you mean you are not receiving the Notification email of your form? I checked the mail logs of the email recipient of your form (jotform@flexcarestaff.com) and I noticed that the emails are sent successfully.
Can you try and check again your form and see if you will receive the email now? If you are referring to a different concern, can you provide us more details so we can further check the issue?
