I checked your account and the forms are not accessible because you were autosuspended.
I see you are a new user, so I have deleted the form fields that violated our terms and reactivated your account. Please do avoid adding or creating similar forms to avoid being autosuspended again.

Many thanks for your quick and clear answer, it's a great help. I did not change anything in the template form, so perhaps it would be wise to warn users of this template (and the likes) that they risk autosuspension when not altering those fields. Perhaps a more specific error code instead of "form not found - this form is disabled" would also save users and support desk a lot of time. Just putting in my five cents :-)
