I am having issues with my forms. I am getting numerous responses related to forms being submitted, but not delivered. Is this on my end or yours?

Is this the correct form in question, http://www.jotform.us/form/52383191929160? If yes, I see no mistake in your configuration. However, the majority of the recipients aren't receiving any email based on our system email logs for your form.
For this kind of problem, this guide should help you fix it: http://www.jotform.com/help/208-How-to-setup-email-alerts-to-prevent-email-bouncing-related-issues. The whitelisting part is important, especially you are using a custom email address in your forms.
