I'm using iWeb to make this site and my form shows up in iWeb perfectly and I've made sure it is published but its not showing up in my browser even after I've refreshed the page. Please help. THanks

Have you tried to re-insert the code to your page? If not, can you re-insert the code into your iWeb page? Here's the guidelines on how to get the code of your form: http://www.jotform.com/help/12-Adding-Form-to-an-iWeb-Page

Ok ... new discovery... My site is being published through my hosting service with Go Daddy. I had a thought that could be a problem on there end because all of my other Jot Forms work on other sites. Those sites are being hosted through MobileMe (Apple) I just published the site in question to mobile me and the Jot Forms work! So it seems that its a Go Daddy issue and I will take it up with them. But if you have any suggestions with this new knoledge please do let me know.

Your form shows perfectly here: http://jbashclarity.com/choice/contact_files/widget0_markup.html, so I'm sure there's something wrong on your form inside contact page, can you please recheck everything on your iweb program? I'm not quite sure about this but this page should be in an iframe and not on script code.
