I would like my choices for many questions to be in columns but I can't get them to be on one line and it makes it look very strange. How can I change this?

    Hi Marisa,

Are you referring to the checkbox field in your form? I checked your form and I noticed that the checkbox field options are now displayed in 3 columns.

If you are referring to a different concern, can you provide which field are you referring to so we can further check? If you can provide a screenshot of the issue then that would be great. Just follow the instructions provided in this article to attach the image here in this thread: How to add screenshots images to questions in support forum.

Do let us know if you need further assistance.
    Regards.

So I ended up having to change some of the options so they would fit. For
example, in the role or witnesses category it used to say
Support/Administrative Staff. The closer I can get it to the paper copy the
less likely mistakes will be made when filling it out online. Does that
    make sense? Thanks!

Unfortunately, I'm unable to understand your concern correctly. Perhaps you can share us a screenshot of what kind of layout you are looking for?

If you would like to position form fields in one row, you can check this guide on how to do that: http://www.jotform.com/help/328-How-to-Position-Fields-in-JotForm

You'l probably need to "Shrink" them.
