    Why don't user-input price and months work on my form? Also want 60 month, monthly subscriptions

    a) The last few "subscriptions" allow user-entered # of months and prices, however, it does not seem to work.

    b) We want some subscriptions to have a 60 month monthly term, it appears that the limit is 52, what shall we do?

    That's because of the following CSS code injected in your form which limits the height of the subscription list.

    .form-product-item {

        width: 100%;

       height: 28px;


    Removing the height  height: 28px; should reveal those fields. You can find it on the CSS codes injected in your form http://www.jotform.com/help/117-How-to-Inject-Custom-CSS-Codes.
