Please help! Form was working fine and now is not.

Are you referring to this form - http://www.jotform.us/form/42965818593168? I checked the mail logs of the email recipient of the notification email of the form (email@example.com) and the emails are sent successfully.
Are you still not receiving the email? If so, I noticed that you have setup a custom Sender Email in your notification email. Can you try to use firstname.lastname@example.org as Sender Email and see if you will now receive the email?
