Am I using JotForm forms on my website?

    I have 2 websites and I was wondering if i am using jotform?

     

    www.ps4repairnj.com

    www.ifixinj.com

     

    Because sometimes I am not receiving all of my quotes and I was wondering if I am reaching my limit?

    I have checked your JotForm account, and you are nowhere near your current account limits.

    You have had 0 submissions in your account this month, and your account is currently using zero storage space. In other words, you don't seem to be using the forms from your yufixit JotForm account.

    I have also checked all the links you had provided, and none of them contain any JotForm forms at the moment. On the page of your third link, http://www.ifixinj.com/quote/ , there is a form that was built in Avia Builder:
