We have customers that come in with fleet vehicles and would like to set up a fleet account so we can invoice them as a company. This is for oil changes, etc. I saw one example (Pep Boys) that had pertinent information but will need some guidance if a template is not available. Please help.

You can try to search our templates and base your requirement on the available forms.

You can also, of course, create your own form through our form builder.

Thank you for your quick response!!! I am a little behind the times so if
you could tell me how to get to an existing form (i. e. Pep Boys Fleet
Application Form) and make changes to my form, that would be a big help! I
looked up the existing templates but couldn't find anything that came close.

If you cannot find any similar templates to your application form on our Templates page: http://www.jotform.com/form-templates/search/fleet%20application, you will need to manually create the form. Here is a guide that might help you in creating your form: How to Create Your First Web Form.
