    Good evening , we require assistance because the form posted online via the link is no longer displayed either within the platform JotForm . We also have the fee paid Username : webadiconsum . We wait your information . Thank you

    I did not encounter any issue and the form was submitted successfully on Preview.

    I see that, you are using IE.

    Please try updating Internet Explorer to latest version or try with different browser.
