How i can add username and password

Do you want to add username and password fields to your form? We don't allow forms with password fields since it violates our Terms of Use. Forms that have username and password fields are considered as phishing and automatically detected by JotForm. 

Phishing You agree that Company may terminate your JotForm Service immediately if a form is created or JotForm is found to be used to: Obtain credit cards sensitive information, get social security numbers, show content with child pornography. Or if a form was made to steal user login credentials. All these activities are considered as Phishing and any account along with the guilty forms will be suspended immediately.

I have inspected your forms and in your form http://www.jotform.me/form/53451753800452  you have a password field.

I have removed that field from your form. Please do not add this type of fields to your form, because your account will be suspended by our phishing system.
