In the forums I read that the fix could be using the iframe imbed method. I tried this with one form by replacing the jot form short code with the iframe imbed code and the result was that the form didn't show up at all on any device. Please help. This issue is affecting about 5 forms on my site and basically makes my site useless.
For the example url below I am providing a link to and screenshot of my locations page, instead of the contact page which I mention above. I am doing this because the contact page, as I mentioned above, now has no form whereas the location page shows a form that is cutoff.

I have checked the link to your form and I'm unable to load it :
But, I would suggest you to use the IFrame method to embed your form, in order to avoid issue, this is the guide to get the IFrame code : http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code

Remove the current code, get the Iframe code again, before you embed the code, paste it in a text editor like WordPad, you will notice two part of codes, only copy and embed the iFrame part in your webpage, example:
Make sure to set enough height so your form shows all, and also set the scrolling property to "yes" if needed, example:
