I could not recall my password, so followed the reset procedure. I still have not received the e-mail resetting it. Username is ISSI. I believe e-mail address on file is either email@example.com or firstname.lastname@example.org. The form ID on my account is 12835025353.
I am receiving the submitted web forms for my form at this e-mail address, and assume the account is registered to the same.
Please advise as to next steps.

The form-id above belongs to a guest account. Means, you weren't logged in as you created the form. When did you start to use JotForm, if i may ask? We'll be waiting for your response. So we can assist you further.
