Our logo is not displaying properly

This has been happening for the last couple days and I can't tie it to any changes that I made to figure out the cause. Can you take a look? We had a white logo in the header of our form, and now it looks like a broken link or something. Thank you!

You need to upload the photo or enter the image's URL again. The reason for this problem can cause from anything; but refreshing the image will fix this problem.
