I do not receive a noticfication email on some forms unless I put in the name of 'Jotform' and email address of 'email@example.com' in the settings. Can this be fixed?

If you want to use the email address of the person who filled out the form, your other option is to have your email service provider whitelist our mail server ip addresses, they are:
22.214.171.124, 126.96.36.199, 188.8.131.52, 184.108.40.206, 220.127.116.11
Unfortunately, those are really your only two options because of the nature of spam filtering.
