Are you referring to your login credentials here in Jotform? I can see that you were able to login to the account, bizken, when you posted this thread. Are you pertaining to a different account?
You can use "Forgot password?" option from our login page, so you can recover/retrieve your username and/or password. If you can't remember your username as well, you can use the email address you've used creating the account.
After clicking "Send Reset Instruction" button, you will receive an email on the email address associated with the account. You just need to follow the instruction on the email sent to you.
