I've gone to the 'my account' page and clicked 'cancel' but it just takes me to a plimus login page and I don't know what username and password I'm supposed to enter here.
Please can you cancel my subscription and confirm via email once this is done?

You have paid Your JotForm subscription using our payment gateway, Plimus. Your Plimus username is a long string of numbers from the original plimus email you received when you paid for JotForm,
Once you found your username, you can enter it on the link below and enter the email address used in email. https://secure.plimus.com/jsp/forgot_password.jsp??designId=1
I have already canceled your subscription for you so you don't have to do the process above. You will not be charged again. Your account will stay Premium until March 2, 2012.
If you decide to subscribe with us again, you may do this anytime.
