My forms have been disabled and I don't know why. Can someone help with this please?

The reason that your form was disabled and your account automatically suspended was due to the passport number and passport expiration fields on the form. We do not allow for collection of sensitive personal information such as credit card details, social security numbers (SSN), or passport IDs.
We have an automatic tool that scans for violating forms and suspends the JotForm account that owns them, which is how your account got suspended.
Please remove the fields for Passport Number and Passport Expiration, and your account should be automatically re-activated. For examples of how our other users are collecting passenger information instead, you can take a look at the forms shared in our Templates Gallery that are for collection of passenger information:
You can collect passenger names, addresses, emails, phone numbers, their citizenship, and other information that is not about their SSN / Credit Cards / personal ID.
