Sorry--I was in a hurry--that does sound silly, doesn't it?  I meant to ask if I can put a jot form on a mobile website desiged for smartphones, i.e., m.xxxxxxxxxxxx.com.

I don't see a reason why not. We have tested the forms on iPhone and Android. Can you tell me what kind of phone browser this form needs to support?

We haven't been able to test it on Blackberry since no one has it on our office. Can you test it and let us know how it works?

I could not get it to work.  Of course, I'm using the program provided by the hosting service so when I copy the source code and try to paste it into the web design program, in copies the code as written rather than an HTML placeholder.

What is the hosting service are you using? Is there a "Source" mode when you edit the page? Can you edit pages by FTP?

APlus.net is the hosting service.  No, unfortunately there is not a "source" mode--it's a very simple insert, paste and publish program.  I will try the FTP edit--thanks!

Yes, that should work. If there is no "source" mode, then the only option would be to edit the files manually using FTP.
