My private Facebook page doesn't show up when trying to add the app

I have a private facebook page I'm trying to add my jot form to but it doesn't come up with I follow the steps. My other pages come up but not my private. How can I get the tab on my private FB page?

If the page won't appear in the dropdown options then that means you have already added the app to it. Would you mind letting us know the Facebook Page URL? We'd like to check it for you. Please note that this will only work for Fan Pages, not group.
