Everyone I send this form to will not open due to concerns about certificate error. would like to upgrade including other forms but want to figure this problem out first. This was with Internet Explorer.
I can't replicate this problem using IE 11. I think the error is on the older version of IE. Your screenshot had failed to appear here so I can't see it. Please re-upload that by following our steps here: http://www.jotform.com/answers/277033.
Also, try re-embedding the form using our iframe method: http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code. Check if this helps.
