Is it possible to add a second checkbox to the short scrollable form?

    The short scrollable form works great. Is it possible to require a second check in a second check box that can also be mandatory, with verbiage next to it that refers to something else within the terms and conditions?

    Unfortunately, it's not possible. You can use another widget such as this: http://widgets.jotform.com/widget/terms__conditions.
