Hello Cinderella,
The URL you have shared is actually a custom code of my colleague @Titus.
In which form you want to add this kind of tabs? Do let us know and we will try to help you with required custom HTML & script code. Please be noted that you will not be able to add these custom code in your form and it needs to be hosted in your own server.
You may also like to take a look at our "Form Tabs" widget and see if that fits into your requirement.

The widget will work with tabs at the top by default:
http://widgets.jotform.com/widget/form_tabs
However, you are correct, the setup you see in that form is quite custom. We can help you apply it to a form if you'd like. Just let us know the form you would like it applied to and we will see what we can do.

Please let us know on which form you would like to add "Form Tabs" widget and we will try to help you. Please note that for form tabs widget to work, it requires page break field to be added in form. Each page of your form will be displayed in different tab. Please check the screenshot below:
