    Are the forms considered secure? We plan to use this for patient paperwork

    What is the process of getting it on our website?

    Where do completed form go once submitted? Emailed? Saved somewhere?

    If saved, in what form are they saved? Jpeg, doc, PDF?

    We would likely be interested in the Bronze package, what is a sub user?

    Are the forms considered secure?

    Yes, they are quite secure.  Here is some information regarding using your forms in a HIPAA compliant manner:

    Where do completed form go once submitted? Emailed? Saved somewhere?

    The completed forms will be stored in your JotForm account and can be emailed if you would like.  The content of the email can be modified to include or exclude and data you'd like.

    They are stored in your JotForm account and are displayed in grid list format from within your account:

    We would likely be interested in the Bronze package, what is a sub user?

    A sub-user is an additional user that you can allow access to view/edit forms and/or submissions, without having to give them access to your account:

    This allows you to have multiple people access to create and edit forms in your account and retain administrative control over all the forms.

    Great news! JotForm now offers HIPAA compliance. This means users in the healthcare industry can use JotForm to collect sensitive patient information through consent and onboarding forms, medical history updates, online bill payments, and prescription refill requests. 

    HIPAA-compliant forms require a Gold pricing plan, which is only $99 a month or a Silver pricing plan, which is $39 a month. A business associate agreement (BAA) is also available upon request.
