Form no good on mobile

Hello, my form is breaking the website design when viewing on mobile. Please see attached screenshot from mobile device. Works fine on desktop, but need help with mobile asap!

I already had responsive set to Yes in preferences, but it's still not working. Could you please check the link on a mobile device and advise? I needed this to go live yesterday
