I am unable to access my JotForms

Usei o formulário de vocês em um site:

e não estou conseguindo vizualizá-lo. Por quê?

I see that your account is suspended due to the imported form titled "media-core® - Serviços de Integração de Sistemas". This form is containing a password field which is not allowed with JotForm and hence the account is suspended. Please refer to our terms of usage for more information on using password fields with JotForm. It looks like the form was imported incorrect URL.

I've now deleted this imported form from your account and activated your account. The forms are now displayed on your website.
