The widget is installed. When I view source code of the web page where the form is embedded I can see the source code for the widget is there.

I'm using "?utm_source=SL2016ALKI" but the data isn't captured in the submission nor sent in notification.
I also have installed the widget "Get Visitor Location" and it IS working as it should.

I took a look at the page and I see that you have embedded the following form: https://form.jotform.com/form/60145340352141 by using js embed code.
What I would suggest is to use the iframe embed code instead since it is very likely that there is some conflict that is causing some issues with the form functionality.

Now I would also like to mention that recently Get Referrer widget got changed a bit and there are now 2 widgets that you can embed - each doing its own thing:
Where the later is working as the first was working before. You do however seem to be after the first - Get Referrer widget, so after embedding the form with the code above, it should work for you right away.
