Hello, I am trying to create a form for verification purposes for my new website but my form is getting disabled. I do realize that this may be due to the fact that I am collecting sensitive data but according to your terms:
You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm including SSL and Encrypted Forms features.
I do indeed plan on using best security practices with your default SSL enabled forms and the added encrypted forms feature.
Please kindly advise at your earliest convenience. Thank you!

We apologize for the inconvenience caused. I have checked your account and jotform http://www.jotform.ca/form/60162773983261 and I did not find any issue with them. The Phishing filter had disabled the form.
I have corrected this now and reinstated both the form and account.
Let us know if issue still pesist.
