Widgets are non-responsive on form?!?!?

Widgets are not showing up.

label for appointment widgets show up but no appointment slots.

I have reviewed the conditions and there are no errors. It was working fine.

Check box shows up for terms and conditions shows up but no terms and conditions link or any text.

What gives?

I have inspected your form on my Samsung Galaxy S4 and Appointment slot widgets is show on my end.

I was not able to find terms and condition widget inside of your form. Did you delete it from your form?

I think that this issue is related to your other thread http://www.jotform.com/answers/747035. There was an issue and widgets were not shown on multi page forms. This issue is now resolved.
