I have asked multiple times without a response.

My form was working fine until about 5 pm today.
On the link please make a selection for type of situation.
On date please select a date that is available and a similar thing will occur.
I need to know what is wrong.
I need to ensure that this will be resolved and not happen again.
Here is page 2 of the form Should show an appt widget not just the appt widget label and a terms and conditions indicatio not just a click box.

right, because another jotform user also in a panic helped me out by letting me know that it had to do with some weird bug with the page buttons so I deleted them and magically the widgets started working again.
You have an issue with the page buttons and the widget. That is a fact
I would like to ask you to open a single thread in regards to the same subject / issue in the future.
That will allow us to assist you best as well as to keep everything in one place.

As well as this one, where on the second thread above: http://www.jotform.com/answers/747035 you were notified by my colleague of the issue being resolved after the same was reported to our developers.
