Can you please indicate the form in question? Are you referring to the form, 43074598816163?
I was able to open this form properly.

This is now fixed. Our engineers had to do a emergency data center switch and there is a replication lag between our data centers which causes a delay to see the changes.
You can either recreate the form, or you can wait for your form to be fixed once the data replication catches up.
