About a month ago I created a contact form that is on our website and can be found here: http://canarylearning.com/support/. When creating it I don't remember having to sign up for an account even though I should have. The form still works which is great, but when I go to jotforms.com it no longer shows up. Is there a way to link the form to my email address firstname.lastname@example.org ? It seemed to disappear after I think your website when down for a little bit awhile ago.
Please let me know if you need any other information and if this would be possible.

The form in your page is associated with a Guest account. If you register an account using the email address "email@example.com", the form should automatically be placed into your registered account. If it is not, let us know you would like the forms moved from guest_53477065389064 to your registered account and we will be happy to move them once you have registered.
