Can I create a form as a front for a mail merge document

I want to allow people to complete a form that then generates a document that they can print off.  To do this I need to use the form to generate a document using mail merge techniques to complete legal clauses. I know this is a bit ambitious, but I want to give it a go.

I want to make it so that the document will not be generated until payment is made, so that it can't be printed or downloaded until then.

The document could either be emailed to the person filling in the form or generated in a different window for them to print off.

Can I do this with jotform? I would really like to use Jotform if possible, but If not can you point me in the right direction so that I can do this.

    Kind regards

    Hello Janette,

    Thank you for contacting us.

It is possible to include PDF submission download link into form Thank You Page. You can also configure custom Autoresponder emails.

Please note that we are sending email notifications and showing form Thank You Page for successful payments only (this is true for Payment Forms), so that it is probably something you are looking for.

However, we will have to see an example of the document to check if that is possible with us.

Hi, I was just about to post this question and realised I had already done so.. an example of the final document would be as below.  I would like the data inside the brackets on the document to be populated by the details entered onto form.  After payment I would like the final document to be sent with an email.

I [insert name] born on [insert DOB] of [insert address] etc etc

I APPOINT [insert name] of [address] as my attorney etc etc

Is there a way I can do this using Jotform?

    thanks ;-)

You can possibly do that into your form email notification settings. For example:

The bold texts inside the email body are those tags of your form fields. Unfortunately, there's no way you can do this to the PDF output of each submissions.

If you want the PDF output, you can possibly do it using Integration. Check it here:
