I am in the process of setting up multiple apartment properties with forms for leasing apartments and this shutdown concerns me. If I am using this service for something that important I need to know that JotForm is not running a risk of being shut down in the forseeable future. And it seems some of my forms are still working with the regular jotform.com domain both on the web and in the editor.
I also have another question about my form http://plusmgt.com/application/fp_app.html
At the very end of the form, in the payment area.. it is set as a required field but is there any way to make both fields required and they just have to select a quantity?

We are currently having a DNS problem with our domain jotform.com. We hope to resolve this issue as soon as possible. As a temporary solution, you can logon to www.jotform.net or www.jotformpro.com. If you have already embedded your forms, please edit your embed code, just change www.jotform.com domain to www.jotform.net.
With regards to your other concern, I just visited your page and found some functinalities of the form that aren't properly working, e.g the form warning. To fix this, please re-embed your form using iFrame code.
To get your form's iframe code, please follow the steps below:
1. While on your form, click Setup & Embed
2. Click Embed Form
3. Choose iFrame
4. Copy the provided code.

all my forms had form.jotform.com instead of www.jotform.com
so can I just change it to http://form.jotformpro.com?
this really is a pain.....being a paying customer too...i have over 75 forms i have to fix now

We deeply apologize for the inconveniences this may have caused you. We only had issues on our www.jotform.com and we are doing our very best to fix this asap. Right now, we recommend every users to use www.jotform.net as a temporary solution while we are still working on this issue. We hope you've understand.
Sorry about that. I just tested your form and it's perfectly fine now. I just forgot to clear my cache.
Going back to your question, sorry to say but it's not currently possible. I can only help you to make it both selected by default but not both required.
To make it both select by default, simply add this at the end part of your form url

yeah I tried addind that ?youMust193=1001,1004 and that didn't auto-select them both. any other ideas?

My bad, I forgot it will only worked on iFrame codes.
e.g. sample iframe code:

My apology. If you'd still like to use this, please re-embed your form using iFrame code. That should solved it. Thank you.

Did you answer about the long-term viability of your company?? I'd like to know that also as I have a lot of forms on your site.

JotForm has been around for at least 6 years. I am not belittling this issue or anything, but this is the first time we had a problem like this. As you may have learned, the cause of the issue was definitely out of our control but we immediately took the necessary steps in order to keep our users' forms working as usual.
As a matter of fact, the JotForm team is planning of taking legal action against the unwarranted domain suspension. You can be assured that JotForm is here to stay. Our ever-growing user base is a clear proof of that fact.
