Code/Links for one of my Forms is not generating properly

My "Bus Shelter Form" is no longer functioning correctly.  The preview and published versions of the form are missing most of its content.  It used to work, and has not been edited since I originally created it, but now it is no longer working.  I have been unable to figure out why on my own.

Sorry for the inconvenience. You may always use the REVISION HISTORY to revert back and undo any changes that may have been done in your form. From your MY FORM PAGE select the form and on the right click on the MORE dropdown and select the REVISION HISTORY.

In the history, you may select the date of when your form was working and revert your form too.
