How to reset the link to my form ?

Hello,

My form is: https://www.jotform.com//?formID=3381414086

Since a few days the link of my form is not good. He is: Toublan/questionnaire-de-satisfaction--sjou

When i reset, it become: 3381414086

so, impossible to send. What happened?

Thank you for contacting us.

You should follow this guide : http://www.jotform.com/help/261-How-to-create-a-Custom-URL-of-a-form 

Then, you will be able to reset the link to your form and then add a new custom URL, you should be able to load your form using both URLs.

If not, please do let us know and share us your link, we should not be able to load it even if the link is in your form.
