When you say not working, meaning, you are getting an error when submitting the form? I have tested your integration and it works.

I just checked it again and found out that some rules in your conditional logic are tied to deleted fields. This will give you issues so I would suggest fixing them up by deleting or re-configuring the logic.
Your authorize.net field is also tied to a condition with missing/deleted fields. I think your problem is related to this.
