Inconsistency Between Forms / Country, State Condition Not Working

    Hello JotForm,

We've replicated one form for our new Whitepaper but it seems like the hidden fields inside the box doesn't show up and even with the conditions of showing States for US and Province for Canada doesn't seem to work as indicated on the website and to validate to the screen shot.  This is not the only form doing this so we're not sure if you recently had an update that is messing up the forms for us.  Please advise ASAP as we're losing leads with the broken forms.

Please let us know if you have any questions.

    Thank you,

    Jasmine

I have just reviewed you site and notice that you are using the source code to embed the form. We recommend using the Iframe code. Please let us know if using this, resolve the issue.

We are using the source code because we add additional codes to the submit button for each landing page.  So using iFrame is not an option for us.  We didn't have this issue before and it just started happening about a month ago.

If you are using the source code to embed the forms, all the modifications made in Jotform do not affect your site. Since you exported all the code, the form it self is being executed on your server. I would suggest you review the code and server setting to verify everything is correct.

Thanks Victor.  We have already reviewed all the codes and the server setting hasn't changed.  As I mentioned before, we only have this issues on some of our forms and not the others.  I have even duplicated one of the forms that works and recreated the landing page from scratch and still have the same issue.  Also the condition for one of our field doesn't work (even though the condition is there).  We didn't have this issue before and came across it a month or so ago.  Is there anyway you could check the code and see if there a bug when a form is being duplicated?

Sorry for the inconvenience. Could you please supply us with the URLs of the other forms that you were able to replicate the issue, so we can verify what you are indicating and see if we can reproduce it in our end. I am also assuming you are using the source for this as well. By the sound of it, it seems that there might be some conflict between the source code of the form and your site.
