How to embed your form with lightbox method

Hi guys,

I want to embed a form into one button that then opens the form on top of the page like a light box does.

Below is a link to one of my sample pages but the form is buggy and I don't have the time to try and fix it but it looks very nice and opens on top perfectly and that is what my clients like.

I looked and looked for a template or theme that starts with a button ant then opens on top of the current page and could not find anything right....

Does anyone know of something close to the way it looks and works on my link above?
