    I assume this was the form http://www.jotform.us/form/51783308563157 you wre having the problem.

    It will help us in understanding the actual issue if you provide more details about it also. In return, we will be able to immediatly provide you with the appropriate response and resolution.

    Can you please tell us what were the issues?

    I test submitted the form, and so far it was sent just fine. You can check the video below when I did  the test submission.
