Our Form was deleted by mistake and I would like to retrieve the form and continue with our previous setup.

I see that there is only one form in your account. If you are referring to a different form, you may restore it from the Trash folder if it is not purged. If the form is purged from the Trash folder, unfortunately, we cannot restore the form.
