I need a way to collect submission data for MySQL database. Your current applications are not working.
I need a solution ASAP. Do you have another option.

The only way to send your submissions data to your database is sending the data to your thank you page, you will find the steps on this guide: http://www.jotform.com/help/51-How-to-Post-Submission-Data-to-Thank-You-Page.
Once you have allowed your form to send the submitted data, you may follow this guide: http://www.jotform.com/help/126-How-to-send-Submissions-to-Your-MySQL-Database-Using-PHP.
That is the correct way to achieve what you want, if you have some issues when doing it, do let us know, we will be glad to take a look on it.

It is a bit late to being doing that now. I have almost 900 submissions already and need that data not new data.

You can try creating an Excel file, I mean, download your current submissions and then import them into your table in your database, I found an article with steps to import the data from an Excel file into a table in MySQL: http://stackoverflow.com/questions/1310166/how-to-import-an-excel-file-in-to-a-mysql-database.
And you may also follow the steps provided above, to send your new submissions to your database.

One thing of note is that I find the reports generator tool generates a much cleaner excel sheet then using the view submissions,excel option.
Same for the csv option there, it can make a very messy file also, mine were useless.
I haven't tried the Reports csv option yet.
Perhaps its just me... Just my 2 cents... wait we don't have pennies in Canada any more so I guess a nickles worth then.
