I Cannot Download PDF Version of My Form

Is there something wrong with the server, I have been trying for over 30 minutes to download my form in PDF formate.  It simply spins in circles stating working.  The other day it only took seconds tonight minutes and hours.

I have been trying since last evening.  Today I tried the form you have above and still cannot it is stuck in working mode.  I need the form 1019 Performance appraisal form.  I don't know what is going on.  I have reset my internet connection and restarted my computer twice.  I have even tried 2 other computers in my home both of which are using Mac platform.  Like I said the other day I had no issues today and last night nothing but problems.  My email is syl.lepine@me.com
