Is there something wrong with the server, I have been trying for over 30 minutes to download my form in PDF formate. It simply spins in circles stating working. The other day it only took seconds tonight minutes and hours.

According to my checks, I have no problem with downloading PDF format of your form on your account. Please, see it from the below:
Alternatively, if you specify the form and give your email address, I can send that PDF file to you. Please, answer this thread with your request, I'm waiting for your answer.

I have been trying since last evening. Today I tried the form you have above and still cannot it is stuck in working mode. I need the form 1019 Performance appraisal form. I don't know what is going on. I have reset my internet connection and restarted my computer twice. I have even tried 2 other computers in my home both of which are using Mac platform. Like I said the other day I had no issues today and last night nothing but problems. My email is firstname.lastname@example.org

Firstly, sorry for the inconvenience. There is an ongoing process about this issue and sometimes, this problem could occur. However, I sent the form which you requested to the email address "email@example.com", so you can use the related fillable PDF.
