For some reason I am unable to upload an image into my form.
I have been able to do it before but it will not work now. I am able to add an image to the header but not in the body of the form.
Can you tell me why this would be happening?
Are there specific dimensions the image needs to be?

I checked to see if adding images to the form was working and it appears to be. I added an "Image" field to the form, then uploaded an image from my desktop:
After I selected the image and clicked upload, it was then added to my form. Any size should work. Does this not work on your end?

No it wasn't working. I have inserted images before and it worked.
This time there is a black box with an X in it and the image won't work.
I thought maybe it was the format of my image, resolution or file type, but I have tried several. It will only allow me to add an image to the header.
The image goes in the image folder to select from but will not add to my form properly.
Any clues as to what might be happening?

Can you try from another browser? If you are using FireFox, try Chrome and visa versa. There may be something preventing your browser for accessing the file. If that does not work, please share a screenshot of what you are seeing on your end and we will be happy to have a look:

Here is the form. I tried in Google and the black box with the X is now a little box icon.

At what point does the box appear? Is the file you are using on your computer? Try adding an "Image" field to your form, select "Upload Image File", select the file then click "Upload file". After you click upload file, the image should appear. At which step is image showing black? Would you mind sharing a screen shot of the issue?

FYI -- Firefox had an upgrade several months ago that has prevented certain websites from loading properly ever since.
This has affected JotForm too in that the image upload tool is no longer working. The upload just hangs. The upload does work in Safari however and probably other browsers.
