I signed up for an bi-annual jotform premium account with payment made via PayPal on 12/7/11, but was still
charged for the monthly payment on 12/11/11 and 1/11/12. Could I get a credit for that please?
Also my account on jotform.net (not sure about .com I haven't needed to log in lately) says I only have a free account not premium.

I have found record of your biyearly subscription purchased last year, but it was for an account 'iaflmturk' ?
