Billing issues, bi-annual account, and account status is wrong on

    I signed up for an bi-annual jotform premium account with payment made via PayPal on 12/7/11, but was still
    charged for the monthly payment on 12/11/11 and 1/11/12.  Could I get a credit for that please?  

    Also my account on (not sure about .com I haven't needed to log in lately) says I only have a free account not premium.

    I have set your account sajego to Premium while we sort through this

    I have found record of your biyearly subscription purchased last year, but it was for an account 'iaflmturk' ?
