How to create a multiple payment form?

I agree. It would be great if you can offer pay online OR pay be CHECK!!!!

Although you can not add more than one payment method directly to your form, there is a workaround, you may use the iFrame widget to show the form  where is the payment method, and also use conditions to show/hide the forms in the iFrame, for detailed explanation, take a look on this guide: http://www.jotform.com/help/182-How-to-Create-a-Basic-Multiple-Payment-form.
