I have a referral form requesting the names of youths. I am requesting the same information for all youth. The form needs to accomodate up to 6 youth, but I don't want to show information for six, if the referral is only for 2 youth.
I have tried to use the "conditions" to control the number of fields that appear depending on the number of youth.
"'How many youth are you referring" - and base the conditions on 1 through 6.
Youth would show a the first and last name fields and contact information (about 10 different fields)
Is there a simply way to achieve this without duplicating all the fields individually?

I can see that you have posted the same question twice on the forum. http://www.jotform.com/answers/770346
Here's the reply I gave on that thread:
The Configurable List Widget may prove useful for your form.

This widget allows the user to add another set of fields by clicking one button. Here's a guide on how to add the widget to your form: How to Add a Widget to your Form
Here's a demo form so you can see the widget in action: https://form.jotform.com/60417580570959
