I transformed my website to one with SSL-protection (www.topdental.nl)
I used to use jotform to subscribe patients in my practice, but nevertheless jotform isn't visible anymore since I use my SSL-protection on my website. What should I do now?

By default the embed code of the forms are now in SSL version.
What I suggest you try now is to re-embed your form on your website. Make sure it was also using HTTPS URL since you claim that your website was alredy using one.
It will also help us investigate further if you can share to us the exact URL of your website where we can check the form used.
