    How can I create a fillable pdf form to save as a file?

    Hello.  My name is Maciek and I have a question that I’m hoping you’ll be able to answer.  I am looking to create a fillable form (preferably in a pdf format) that I will be able to save as any other file on my computer.  The form is a questionnaire (3-4 pages long) which will feature some questions with short fillable fields (name, address, etc.), some multiple-choice type questions with one answer (check one that applies), some multiple-choice type questions with several answers possible (check all that apply) and some questions with larger fillable boxes for longer type-in answers (a paragraph or several sentences).  I will be emailing this form to my clients who will then complete it, save it, and email it back to me at their convenience.  The form will be identical for all my clients so I just need to create it once and save it on my computer.  It is important, however, that my clients will be able to open it, complete it, and save it without a need for any specialty software (other than Adobe Acrobat if it is indeed in pdf format).  I found your website by searching for a fillable form creator and I am wondering if I could create the above form through your site.  If so, how would I go about doing it (would I need to sign up for one of the plans?  If so, which one?  Etc.)  Any information with regards to this would be greatly appreciated.  In case this is not something I could create through your site, I would appreciate if you could recommend something else should you know of anything that would help me accomplish this task.  Thank you very much in advance and I’m looking forward to your reply.

    Welcome aboard!

    First, we suggest to signup an account with JotForm so you can start creating your forms with starter (free) plan limits.

    So basically, you have to create a form first on the form builder www.jotform.com then convert your web form into a fillable PDF Form. Follow this guide: http://www.jotform.com/help/212-How-to-Create-a-PDF-Form

    PDF form works similarly like any other PDF. The user has to open the PDF using of course any PDF software like acrobat reader which we recommend the most, then they can start filling in the form fields. What's so special about JotForm's fillable PDF is that your users can submit the form straight on the PDF by clicking the submit button that is included on the PDF. So once they submit the form, you will receive an email notification that contains their data.     Fillable PDF is available on any plans, free or paid so it is up to your preference if you want to upgrade or not. However, we have submission limits on every account types.
