Form not showing up in "My Forms"

I created a form awhile ago that I can access as a visitor, and is sending data to my integrated email list, but I cannot see it under "My Forms" to edit it.  How can I fix this?

You must have created the form http://www.jotform.co/form/60297212180853 whle NOT logged in to your username AlwaysRising  .

I found the form created using Guest account only. I have now corrected this and transferred the form to your account.

You may need to relogin to be able to see the form in your My Forms page.
